Starting Your Application
Apply for admission to the University of Saint Francis for free using our online application or by obtaining a paper application or at the Admissions Office and submitting via mail. Please be aware that, due to increased processing costs, paper applications must also be submitted with a $20 application fee.
To submit a completed application to USF for consideration, follow these five steps. Feel free to print this page out for future reference.
Step 1: Review USF’s Admissions Requirements
Please review the admission requirements of the university before completing the online application process.
Step 2: Gather Your Personal Information
After you have confirmed that you meet the university’s requirements, assemble the following items before you begin the application process. The online application process is designed to be completed in one continuous session. Information submitted to the university is encrypted using SSL technology to enable secure data transfer and stored in our protected university database.
- Full name (including maiden name, if married)
- Social Security Number
- Date of birth
- Permanent address
- Entering status (Freshman/Transfer/Readmit) and semester you would like to start
- Your major or course of study, if you have decided on your major
- Housing preference
- ALL schools attended along with date of attendance and degrees/certificates earned, including college courses taken while still in high school
- Citizenship
- Family information, including names of immediate family who have attended the University of Saint Francis
- If you are a veteran, you also supply your complete DD214.
- If you have been convicted of a felony or misdemeanor, please prepare and submit a written explanation of the incident(s), as well as supplying two professional references.
- You will be asked optional questions regarding your ethnic background, martial status, gender, and religion. The answers to these questions are compiled for statistical purposes only, and will not affect the evaluation of your application.
Step 3: Request Other Necessary Documentation
While you are gathering your personal information, you need to request other information provided by external sources. You will need to have an official transcript sent to the university from each educational institution you attended, including high school. Please make certain your high school transcript includes your rank in class, grade point average and SAT or ACT test results. You can use our Transcript Request Form and print as many copies as needed, or contact your institution directly. Many high schools can now transmit your transcript electronically. If you plan to enroll in the Physical Therapist Assistant program, you must also submit two (2) references using the university’s Reference Request Form.
Step 4: Complete Your Application
You are now ready to complete the application. Use the information you gathered in steps 2 and 3 to fill out your application. Online applications are designed to be completed in a single session—at this time, you cannot save your progress on your application and return to it later. If applying online, simply press the “Submit” button at the bottom of the application to send your application. Paper applications, along with the $20 application fee, should be mailed to the University of Saint Francis, Admissions Office, 2701 Spring Street, Fort Wayne, IN, 46808.
Step 5: Complete Other Necessary Documentation
In addition to the application, certain programs require other supporting documentation.
- If you are planning to enroll in any of the university’s healthcare programs, you must also complete a Technical Standards Form.
- If you are planning to enroll in the Physical Therapist Assistant program, you must also complete a PTA Directed Essay.
- Applicants to any other major are encouraged to submit an essay to enhance their application. If the essay is written in Microsoft Word, it can be submitted as an email attachment to admis@sf.edu. Make sure your name and address are included at the top of your essay. In this essay, please cover the following topics:
- Why you chose your intended major and what you know about the profession
- Why you wish to attend the University of Saint Francis
- Why you feel you would be successful at the University of Saint Francis
